Please create an informed consent form that future clients could complete after you are fully licensed. Be sure to incorporate materials from the course into the final product. Remember to explicitly state the “mays” and “musts” of confidentiality, as they are stated in the Texas rules and regulations. You are allowed to reference outside sources, but you must give credit to those sources.
1. Write out your own informed consent document. It is okay to use information from other sources if the source are appropriately references and cited. If you do not cite your sources it is considered plagiarism and will be treated as such.
2. You must adhere to Texas LPC rules and regulations, and ACA ethical standards when creating the informed consent.
3. The informed consent must look professional and be functional as a form that clients should complete.
4. Adhere to APA formatting and be sure to cite any references.
5. Pay special attention to synthesizing information from the course into your response. Specifically, any topics related to human sexuality.
