M8 Written Assignment – Final Project: Your Plan for Community Action
NOThis may be a problem that has been long-standing but has not been addressed effectively or it may be an issue that has been newly identified. You may address a problem in your town/city or in your county.
You will use the elements of the MAPP model to explore ways to understand the underlying structure of the community health issue you have identified and develop a specific plan for action that improves the health and quality of life in your community.
Your successful completion of this project will require that you access current community health information about the problem. Specific data and other resource information should be available from your local health department and other local government or non-government agencies including those listed on the CourseTE – You should present your Final Project by addressing these points in list format. However, make sure that you use the MAPP model as your guide in addressing them.
