Building a Plan for Service and Referral System

After viewing the videos create a post that begins the process of designing a plan for service:
- Identifies what services (local agencies) in your community could be helpful for the people in the videos.
- If you are familiar with the agency somebody else has mentioned, please share your experience.
- If you are referring to a local agency, please mention the location/region it services.
- How could you assist the consumer in applying for these services?
- By contributing to this discussion, you are helping each other build a referral system.
- Next, discuss the case manager’s role while a client is being served by another agency. How is confidentiality addressed when a client is being seen by outside agencies that we have referred them too.
- Be sure to explain how specific goals help define the expectations of treatment for both the client/consumer and the case manager.
